First Appointment
During the first appointment, I will ask about the problem(s) for which you are seeking help. I will inquire about the history of these problems, including their triggers, the circumstances that aggravate or alleviate them, and the strategies that you have employed to resolve or cope with these difficulties.  I will also ask about any mental health services you have received in the past and whether or not those services were helpful.  I will provide you with a number of recommendations, explain the policies and practices of my practice and give you the opportunity to ask any questions you might have.  All of my appointments are confidential and generally 45 minutes in length.

Listed below are the registration forms you should download, complete and bring to your first appointment.  If you do not have access to a computer they will be given to complete just prior to our meeting for the first appointment:

  • Patient Registration Sheet.
  • Authorization to Secure Payment.
  • Provider-Patient Agreement.
  • Child and Adolescent Social History (clients ages 4 years to 18 years) or Adult Social History form.

You should consider bringing to your first appointment the following:

  • A list of any questions you might have.
  • A list of your current medications.
  • Anyone, such as a family member or significant other, who is involved or knowledgeable about the problems that led you to seek help or who can provide you with needed emotional support.  
  • The names, addresses, and phone numbers of anyone you might want me to contact on your behalf.  These persons may include your primary care physician, previous therapist or your child's pediatrician, teacher or school social worker.



 Second Appointment​

​In the second appointment we will formulate a treatment plan, an agreement that defines the specific problems you wish to address and the goals that you wish to accomplish. To help you achieve these goals, we will identify your strengths and decide upon a treatment approach, perhaps either psychodynamic, cognitive-behavior (CBT), exposure and response prevention (ERP), humanistic, or family systems psychotherapy. Where indicated, I will refer you to other specialists, support groups, and/or community resources that may help you achieve your treatment goals.
